NURSE PRACTITIONER #08-008                Part Time                          $31.86/hr. - $38.25/hr.

Responsibilities:  Under Physician approved protocols, the Nurse Practitioner provides direct primary care services to managed care and other patients of various ages at the Willow Community Health Clinic of the Health Department.  Provides services in the area of Family Practice, Primary Care and Family Planning and related health issues.  May provide telephone triage and consultation to patients after hours and on weekends on a rotational basis.  Will provide health education and counseling.  Serves as a consultant to other nursing staff.  Performs other duties as directed.

Essential Job Functions: With or without accommodations, this employee will be required to visually inspect and review patient's charts and physical condition upon examination.  Must be able to verbally interview and audibly hear responses from patients.  Must be physically capable of carrying out all elements of a medical physical examination.

Degree/Experience:  Must be a  R. N. licensed in the State of Michigan and additionally hold the specialty certification  as a Family Nurse Practitioner.  Must have completed a Masters of Science in Nursing (MSN) or have completed all Masters level  classroom and clinical requirements for the MSN degree.  Candidates will have a 24 month period to complete all remaining degree and state licensure and certification requirements.  Must submit college transcripts if claiming college credits. 2 years general nursing experience required.  Strong physical assessment training, skills and abilities required.   Reliable transportation required.  Please submit your Michigan Nursing license # and expiration date with your resume and application.
