COURT FORMS

This page refers you to the web page maintained by the Michigan Supreme Court, where they have available many of the approved court forms.  You can complete these forms on line and print to your own computer's printer, using a .PDF format.   Once you complete the form and print it out - bring that to the Court for filing.   The clerk will make the additional copies necessary from your original.  If you have any questions about completing the forms, you should contact your attorney.

You may also obtain some of the court forms from our clerks office.   The court will charge a nominal fee ($1.00) for each form.

The .PDF format requires you to have installed on your computer, Adobe Acobat Reader, which will allow your printer to print out the form as it appears on your computer screen.  There is a link below from this page to the Adobe web site, so that you can download (free of charge) the Adobe Acobat Reader.

FOREIGN LANGUAGE FORMS
