RESOLUTION TO AUTHORIZE 2004-2005 AGREEMENT FOR THE OPERATION OF THE OTTO SCHOOL BASED HEALTH CENTER

RESOLUTION #04-330

WHEREAS, Ingham County operates a community health center located in the Otto Middle School, Lansing School District; and     

WHEREAS, the State of Michigan has provided financial support to the Otto Health Center since its inception in 1995; and

WHEREAS, the State of Michigan proposes to provide support through the Michigan Primary Care Association during Fiscal Year 2005; and

WHEREAS, the State of Michigan funds to support the operation of the Otto Health Center are included in the Health Department’s 2005 budget; and

WHEREAS, the Health Officer has recommended that the Board of Commissioners authorize the proposed agreement with the Michigan Primary Care Association.

THEREFORE BE IT RESOLVED, that the Ingham County Board of Commissioners authorizes an agreement with the Michigan Primary Care Association to provide financial support to the operation of the Otto Health Center (Otto School Based Health Center).

BE IT FURTHER RESOLVED, that the agreement shall facilitate a State contribution of $175,000.

BE IT FURTHER RESOLVED, that the period of the agreement shall be October 1, 2004 through September 30, 2005.

BE IT FURTHER RESOLVED, that the Board Chairperson is authorized to sign the agreement after review by the County Attorney.
